Completed application form, complete pre-entrance exam, high school diploma/GED and CNA completion, three references, completed physical exam, completed personal health form, drug screen. $70 application fee is due with the application and $75 testing fee is due at the time of the entrance test. Students have 3 chances to pass the entrance test in a lifetime.
