Program Description
Chicago State University (CSU), Department of Public Health, offers an 8 week, Community Health Workers (CHW) Certificate program to individuals interested in gaining a knowledge and understanding of promoting health and improving health outcomes. Participants will be trained in identifying health disparities and bridging the gaps in access to care in diverse communities, provide outreach education, health advocacy,care coordination, support and cultural competence and collaboration between health care providers to foster healthier communities. After successful completion of the program participants will earn a certification as a Community Health Worker with the ability to apply for employment as a Community Health Worker or Health Navigator.

Application Procedure
The application process includes determining the eligibility of the prospective student (must be at least 18 years of age and a legal resident of the Unites States), complete the online application (which includes paying the fee for the program OR applying for tuition assistance if available), after recieving an acceptance to the program by letter and/or email and via U.S Mail, participants will be required to attend an orientation to the program either online or in person prior to the start of the course.
Participants who desire to use the WIOA grant must visit your local agency to obtain a Career Counselor. Maximum Tuition and Fees cap for this occupation is $5000.00
