July 01, 2015 -
  • WIOA Transition 4 Form 002 was updated to allow for a extension number as part of the phone number field. also replaces O*Net Code with NAICS code to identify the industry of the business representative  
July 14, 2015 -
  • The Average Annual Receipts field has been removed. If the individual is to be identified as a representative of a small business, the determination must be made utilizing the U.S. Small Business Administration, Size Standards Table and must be checked on the form using the checkbox to the right of the NAICS Code.
  • By virtue of signature on the form, the appointee/reappointee is acknowledging that they represent a small business if they have checked the appropriate checkbox.
  • If an older version of the Appointment Reappointment Form 002 has been completed by an individual, it will be accepted.
  • Appropriate changes have been made to the Composition Summary Form 003, older versions of this form will NOT be accepted as they may not have accurate formulas and data validation. The current version should be July 2015 v 1.2 as listed in the footer.
  • The second tab of this form has been relabeled as the WIOA LWDB Membership Profile. It populates data from the Composition Summary Form.
July 23, 2015 -
  • An additional Conditional Formatting change was made to Form 003 as reflect by version 1.3. NOTE: If you have completed this form on version 1.0 or 1.2, they can still be used, however the conditional formatting which highlights an error in meeting the requirements for at least 20% membership under the Workforce/Labor category will not properly be reflected in rows 57 and 58, however, they are appropriately indicated in rows 7 and 8. The previous versions of the form can be used, but should not be submitted without all errors addressed as identified in rows 7 and 8 (ignoring the error messages in rows 57 and 58). If you wish to have your current form updated, you can send it to mark.a.burgess@illinois.gov with a request to correct the conditional formatting.
